Appointment Requests and Patient Portal Registration Requests
Use the interactive form to (a) request an appointment or (b) request a link to the patient portal registration (select from the Type of Request drop-down box).
- Appointment requests: Someone from our office will respond within 48 hours.
- Patient portal registration requests: Once your request has been reviewed and processed, you will receive an email with a secure link allowing you to register for our patient portal. For technical issues, email us at firstname.lastname@example.org.
If you prefer, you may instead call us at (404) 589-2670. For your first appointment, be sure to bring any diagnostic films or other test results that may have been ordered or performed by another physician.
